* If we participate with your insurance company, co-payments
and coinsurances are due at the time of service.
* If we do not participate with your insurance company, we cannot
bill them. However, you may contact your insurance company to
see if you are eligible for “out-network” benefits. With “out-
network” benefits, you pay our office at the time of service
and we can provide you with a receipt, which you can submit
to your insurance company for reimbursement.
